Davenport Community School District and Immunizations
The Iowa Immunization Law requires that parents of all children attending schools in Iowa provide written proof of immunizations. No Child May Attend School without a Certificate of Immunization, except under circumstances outlined or specified in the State of Iowa laws or codes.
Immunizations are state required:
- entering Kindergarten (ages 4-6): Completion of 5th dose DTap, 3-4 doses Hib, 4th dose PCV13, annual dose of Influenza, 2nd MMR, 2nd Varicella, 2 HepA, 3 doses Hep B, 4th dose Polio on or after age 4 if born before September 15, 2003.
- 7th grade or above: 1 dose of meningococcal vaccine (A,C,W,Y) on or after 10 years of age if born after September 15, 2004 and a Tdap booster.
- Twelfth grade: 2 doses of meningococcal vaccines or 1 dose if received when the student was 16 years of age or older.
*These vaccines must be done before school begins and proof brought to the nurse. IDHHS is mandating that there will be NO exceptions for lack of immunizations at the 7th and 12th grade levels for vaccines upon entering school, unless there is a religious exemption or medical exemption on file for the student:
