NOTES FROM THE NURSE’S OFFICE

Dear Parents and Guardians:

     Hello!  My name is Janice Telsrow and I am the School Nurse at Wilson Elementary.  I am full time at Wilson this year.  Please feel free to contact me at 563-723-6950 with any concerns, questions or any health information about your child.  Below you will find information about visits to the nurse’s office:

Please notify the Nurse’s office –

1. If your child has a change in health status, a communicable disease, needs to take medication at school, or has developed a chronic illness that is diagnosed.

Please notify the elementary office – 

1. With any changes in addresses, phone or emergency numbers or contacts.

2. If your child will be absent, please give 2 physical symptoms or physician’s diagnosis on what is keeping your child home ill.

Fever:  If your child’s temperature is 100 or above, you will be called and the child will need to go home.  Children are to stay home until they have not had a fever for 24 hours and have not taken Ibuprofen or Tylenol for 24 hours before returning to school.

Headache or stomach ache:  If your child is running a temperature and has a headache that is severe or has a stomach ache, you will be called.

Vomiting and/or diarrhea:  Your child must go home.

First Aid:  will be given for minor cuts, scrapes or bruises with no call home.

Injuries requiring more than First Aid treatment:  You will be called and your child may have to go home.

Rashes:  If your child develops a rash that is not localized (in more than one area), has drainage, or is causing discomfort, you will be called and your child will need to go home.

Eyes:  If your child has one or both eyes swollen, discharge, itching, or is painful.  You will be called and your child may have to go home.

Medications:  All prescriptions and nonprescription medications must have a Davenport Community School District Medication administration form.  You can obtain these from the nurse’s office and I can explain the procedure for your child’s medication needs.

**Students should remain home when ill and return when symptom free for 24 hours (no fever, vomiting, diarrhea, etc.)**

Health Screenings:  Hearing screens are done by the Area Education Agency for grades K, 1st, 2nd, 5th.  Vision screenings are done for grades K, 3rd, and 5th.  I will send a note home with your student if there are any abnormal results and help with referrals if needed.  

Head Screens:  Parents/guardians are responsible for checking their child’s hair weekly (recommended).  If lice/nits are found, please treat and notify the nurse.  Any information obtained is confidential and I can help answer any questions you may have.

Breakfast/Lunch:  Please send healthy foods for sack lunches and snacks.  Breakfast and lunch can be provided by the school.  If you need any information on healthy food options, please contact me. Breakfast and lunch are FREE this year at Wilson Elementary through grant funding.  A healthy fruit or vegetable snack is provided during the day at school everyday except for Tuesdays through a grant also.
