Preschool Requirements at the Villages

The Children’s Villages house children between ages 3 to 5 years.

Preschool Requirements at the Villages:

  • Proof of a current physical (done within a year of registration) on file within 30 days of admittance to preschool. The physical must be done by an M.D., D.O., N.P., or P.A.   A physical by a Chiropractor is not acceptable. Students need an updated physical on file every year that they are enrolled.
  • Proof of current immunizations for their age group as regulated by the State of Iowa before they attend school.
  • A medical provider (for emergency purposes)
  • A dentist (for emergency purposes)

Use the 24 months or older guidelines for preschool children, not the 4-year-old elementary guidelines. A few differences between Preschool and Elementary Requirements include:

  • Hib is required in preschool; it is not indicated for persons 60 months of age or older.
  • PCV is required in preschool; it is not indicated for persons 60 months of age or older.
  • Hepatitis B is not required in preschool; it is required in elementary/secondary.

Vaccines recommended for the 4-6 year group includes:

  • 5th dose DTap
  • 4th dose Polio
  • 2nd dose MMR
  • 2nd dose Varicella
  • Annual influenza vaccine

Other Recommendations:

  • Lead screen at 12 months old then every year after that through 6 years old.
  • Dental screen by age one, and then every year after that.
  • Vision screen before 3 yrs of age and upon entering Kindergarten and third grade.

Kindergarten Roundup and School Registration

The focus of the Davenport Community Schools Health Services Program is to promote the highest level of health for the school-age child through preventative services such as health education and counseling, health screenings and assessments, and provider referrals. Our program is based on the knowledge that a healthy body, healthy mind and healthy living habits are essential in achieving educational potential. Kindergarten is an important milestone, and we wanted to share some health information specific to your student. We strongly encourage that Kindergarten students receive a physical before school starts, but there are also some required documents/screenings that you should know about.

Forms required by state law:

  • Blood Lead Testing – We do not need the results, just need to confirm the test has been done.
  • Proof of Dental Screening-Your child should see the dentist for this screening Dental forms must be on an Iowa Department of Public Health Screening Certificate – Certificate of Dental Screening 8-17-16 (download below)
  • Proof of Vision Screening- Green Vision cards will be in the Health Offices; as well as the Certificate of Vision Screening form. Certificate of Vision Screening (download below).
  • Kindergarten Enrollment and Emergency Health Information Form.

Below is a list of forms available upon request:

  • Certificate of Immunization Exemption/Medical and Religious
  • Guidance for Students with Special Dietary Needs (This form is completed by a physician if it is for students with food allergies or food texture modifications.)
  • Medication Administration Form (This form is completed by a physician for meds that have to be given at school.)
  • Certificate of Blood Lead Testing Exemption for Religious Reasons
  • Critical Care Cards and epi-pen waivers for students with special medical needs who use the bus system.

We strongly encourage all kindergarten students to receive a school physical by a licensed physician before entering school.

Hawk-i is a state program that offers low-cost or no-cost health and dental insurance for children in families with incomes too high to qualify for Medicaid, but too low to afford private family coverage. Application is available upon request or contact Care for Kids at (563) 328-4114.
