If medication must be given at school, the parent must provide a written and signed doctor’s order stating the name of the child, the name of the medication, the hour to be given, and the dates the medication is needed. Please use the Medication Administration Consent (download below) for the prescribed medication. Whenever possible, the parent or guardian is the accepted person to bring the medication to the health office. Medications should not be brought to school by students. All controlled medication needs to be immediately counted by the health office staff and the person bringing in the medication upon receiving the medication. Please DO NOT bring in more than 30 count of medication at a time.
If your child is on an over-the-counter medication, please give it at home, alert the health office staff and document the time of dosing, so they can watch for adverse side effects of the drug. However, if an over-the-counter medication needs to be given during school hours, the school health office staff must receive the medication in the original, sealed container that will be kept and left in the health office until the parent or health staff deem it unnecessary to continue administration. Cough drops should be sent only if absolutely necessary. If your child has a frequent disruptive cough, please keep the child home. If cough drops must be sent, they must be in the original package with a consent for over-the-counter medications form, and they must be kept in the health office.
A student, with authorization from the health care provider and parent/guardian, may carry and self-administer an asthma inhaler and/or auto-injectable epinephrine medication. The authorization consent, where the physician and parent/guardian need to sign, is on the bottom section of the Medication Consent Form.
Please make arrangements to pick up all medication any time the medication is no longer in use. Please call the school health office to make arrangements. No medications will be kept over the summer months, and proper disposal will take place following the last day of school before summer break in conjunction with the Davenport Police Department.
Any unlicensed person administering medication for the student will complete a medication administration course and skills reviewed by the school nurse prior to administering medication.
1 adult and 1 junior epinephrine pen will be maintained in all school buildings except for the Children Villages under DHS governing. Only fully trained medication administration personnel are authorized to administer this medication in the school or on school grounds. (Stock epinephrine pens will NOT be on field trips as they are needed in the buildings.)
