Davenport Community School District Diabetes Policy
The majority of DCSD diabetic students are patients at the University of Iowa. Therefore, the DCSD health services utilizes all forms from the University of Iowa to provide the best possible care to our diabetic students. Please call your school health office for the necessary medical forms.
Medical School Forms
- In an effort to complete necessary medical school forms before the beginning of next school year, we need to begin planning now
- Before the end of this school year, please ask for medical forms required by your school
- Parents and student are responsible for completing all information, other than medical information, BEFORE giving the forms to your Diabetes Health Care Team
We will not accept BLANK FORMS & WILL RETURN all blank forms sent to us. AFTER PARENTS HAVE COMPLETED THE FORMS, please send them to us for review and signature before AUGUST 1.
Examples of forms that may be needed are:
- Medication Authorization Form(s)
- Individual Health Plan/Disease Action Plans
- Athletic Physical Forms

Forms will be returned to school or home by fax or mail within ten business days. If you want us to mail it, please include a stamped self addressed envelope.
