Required Registration Documents and Health Paperwork

Required for:

• all new students to the district, including new students added to a current household
• current students who have a change in address

An acceptable document:

• must be current and include a date
• must include the name and address that matches the name and address of the parent or legal guardian of the student(s) being enrolled

Please provide one of the following:

If you are a HOMEOWNER:

• • mortgage statement from last or current month
  • utility bill from last or current month
  • most recent tax receipt

• if recently closed on home, settlement statement from the closing OR warranty deed

If you are a RENTER:

• current rental or lease agreement
• utility bill from last or current month

If you are a CO-RESIDENT:

• bank statement OR pay stub from last or current month
• completed and NOTARIZED verification of residency for the family with whom you are residing with

If none of the above describe your current situation, please contact your building secretary. 

Required for:

• all new preschool students
• new kindergarten students NOT previously enrolled in the district’s preschool program
• new students in grades 1-12 attending school in DCSD for the first time

Please provide one of the following:

• birth certificate preferred
• passport or signed immunization records acceptable if needed

Required for:

• all preschool students
• all student-athletes annually

Proof of a current physical (done within a year of registration) on file within 30 days of admittance to preschool.

The physical must be done by an M.D., D.O., N.P., or P.A. A physical by a Chiropractor is not acceptable. 

Students need an updated physical on file every year that they are enrolled

Proof of current immunizations or immunization exemption for student age group as required by the State of Iowa before they attend school

Required for:

• preschool, kindergarten, grades 7 and 12
• students attending school at DCSD for the first time

NOTE: Use the 24 months or older guidelines for preschool children, not the 4-year-old elementary guidelines. A few differences between Preschool and Elementary Requirements include:

• Hib is required in preschool; it is not indicated for persons 60 months of age or older
• PCV is required in preschool; it is not indicated for persons 60 months of age or older
• Hepatitis B is not required in preschool; it is required in elementary/secondary

Additional Vaccines required for kindergarten or students attending DCSD for the first time includes:

• 5th dose DTap
• 4th dose Polio
• 2nd dose MMR
• 2nd dose Varicella
• Annual influenza vaccine

Additional Vaccines required for 7th grade includes:

• Tdap
• 1st dose of meningococcal

Additional Vaccines required for 12th grade includes:

• 2nd dose of meningococcal

Required for:

• kindergarten and grade 9

NOTE: dental screenings are provided at school for students in kindergarten. Grade 9 dental screening must be completed by a dental hygienist or dentist. 

Required for:

• kindergarten and grade 3

NOTE: vision screenings are provided at school in the fall for all students in kindergarten and grade 3, which fulfills this requirement

• Lead screen at 12 months old then every year after that through 6 years old

• Dental screen by age one, and then every year after that
• Vision screen before 3 yrs of age and upon entering Kindergarten and third grade
