The following documents are required for staff to process your students registration:
- Proof of Residency
- Proof of Birth
The following required paperwork must be submitted:
- Physical Examination
- Immunization Certificate or Certificate of Immunization Exemption
- Dental Screening Certificate
- Vision Screening Certificate
