Welcome to Williams Intermediate School. All information regarding health concerns/needs for students are in their school planner. However, I would like to review a few very important rules: 1. Please complete the Health Emergency Form with accurate information by the first day of school. Emergency phone numbers can be critical in an emergency. If information on this form changes throughout the year, please call me. 2. ALL medication must have a physician’s order, be provided by the parent and kept in the nurse’s office. This includes over-the-counter medication. I cannot give your child medication without a doctor’s order. There are many reasons for this and if you would like to discuss this further, please call me. 3. A parent can excuse their child from gym ONCE a quarter. All written excuses from gym after one time, MUST have a doctor’s excuse. Also, the doctor must write permission to allow the child to resume gym. Please call me or the gym teacher with questions. 4. The general rule to send a child home sick from school is a temperature over 100 or vomiting or if the child is in pain or distress. There are many exceptions and each child will be dealt with on an individual basis. 5. The nurse does not call home EVERYTIME the student comes into the nurse. A daily log is kept and if there is a concern or pattern of behavior noted, the nurse will call home. 6. If your child is ill or suffers an injury at home, it’s the parent’s responsibility to seek medical attention BEFORE sending them to school. Ice will NOT be provided for home injuries without a note from the doctor. Barbara Taylor RN
