Requirements For a Certificate of Immunization

1. Polio

Three (3) doses of oral vaccine are required with at least one (1) dose received after the child's fourth (4) birthday.

2. DPT

Three (3) doses of DPT vaccine are required with at least one (1) dose received after the child's fourth (4) birthday.

3. Measles

One (1) dose of measles vaccine after the age of twelve (12) months, and if the vaccine was given after Feb. 1, 1981, it must have been given after the age of 15 months. A second dose of measles vaccine is now required for all children entering kindergarten after July 3, 1991. The first dose shall have been received on or after 12 months of age, and the second dose shall have been received no less than 30 days after the first dose.

4. Rubella

One (1) dose of rubella vaccine after the age of twelve (12) months, and if the vaccine was give after Feb. 1, 1981, it must have been give after the age of 15 months. A second dose of rubella vaccine is now required for all children entering kindergarten after July 3, 1991. The first dose shall have been received on or after 12 months of age, and the second dose shall have been received no less than 30 days after the first dose.
