Message from school nurse for 12th grade students

For the upcoming 2017-18 school year all students entering, advancing, or transferring into 12th grade and born after September 15, 1999, will need proof of two doses of meningococcal (A, C W, Y) vaccine; or 1 dose if received when the student was 16 years of age or older. This vaccine must be done before school begins in August and proof brought to the nurse. State is mandating that there will be NO exemptions for lack of immunizations at the 12th grade level for vaccines upon entering school.

There will be no grace/extension period for the implementation of this requirement. Students not meeting this requirement may not begin the school year until this has been met. Please keep in mind that there must be at least an 8 week interval between the two doses.

For more information you may call the Scott County Health Department at 563-326-8618 or visit www.scottcountyiowa.com/health. You may also call the health office with any questions at 563-336-7600.

    Important Information for Incoming Freshman

    Incoming 9th Grade Students (Parents/Guardians):

The State of Iowa requires that ALL 9th grade students have a Certificate of Dental Screening form on file in the nurse’s office within 3 months of the start of school (by November).  This screening cannot be more than one year old, so schedule your visits now.  Your Dentist should have this form in their office however you may also pick one up from the school nurse.  Please complete all boxes at the top of the form under STUDENT INFORMATION.  Your Dentist can fax this completed form to Mid City High School Attention School Nurse at 563-391-0357 or feel free and call Kristen Swisher, RN (School Nurse) at 563-336-7600 to see if a certificate is on file in her office.  You will continue to receive this reminder whether a form has been sent or not.  I apologize for the inconvenience.

United Way Sponsored Born Learning Session #4

Mrs. Blinkinsop, school librarian, reached out to United Way to have all of the Teen Mom’s at Mid City High School participate in the grant funded Born Learning workshops.  This is the fourth session and United Way volunteers distributed donated items to our Teen Moms that were registered and/or attended. Thank you United Way for being a valuable community partner.
