Process for a obtaining a school permit:

Print and complete an Affidavit for Minor School License from the Iowa DOT website.  Bring the completed form to Brenda Thie at the ASC along with the student’s driving permit, which must be in the student’s possession for at least six months, and the certificate of completion from the Drivers Education course.

Once all parts of the process are satisfied, Dr. Tate will sign the affidavit. The final step is for the student, accompanied by a parent, to go to the Driver’s License Bureau for issuance of a new driving permit.

Please call Brenda Thie at (563)336-5083 or email her at thiebren@davenportschools.org.
